Shotgun Start at 1:30 - Dinner and Prizes to Follow

Entry Fee: $200 per player. Includes, golf, dinner and prizes.

All proceeds from this tournament go towards providing addiction treatment for individuals who can't afford it.

To Enter: Print the registration form below and email the names of players in your foursome to: golf@recoverycalgary.com

Tournament fees must be paid at time of registration.

Fees can be paid via: email transfer, the PayPal button to the right or by providing credit card information in your email registration. If you choose to pay by credit card with registration, please provide card number, cardholder name, expiry date, security code from the back of your card and the billing address postal code.

Please pay for your foursome if possible.
